General Grants – Funding Application Form

The 2009 funding application is now available. Please read criteria before submitting application. Application must be received no later than Sept. 9, 2008, by 5 p.m. Central time.

To apply for funding, please complete the funding application form and submit the completed application along with a set of required attachments, including ALL of the following:

1. Copy of IRS 501(c)(3) designation letter
   
2. A brief history of the organization
   
3. Budget outline of the project for which grant is requested
   
4. Detailed organization budgets for previous two years
   
5. Most recent audit
   
6. List of Board of Directors of your organization
   
7. List of agency paid staff positions and roles (no job descriptions, please)
   
8. Two self-addressed postage applied envelopes to be used for notification

This application form must be typed and completed in full. Incomplete applications will not be accepted. You may print this application and complete it, or you may retype the application in its entirety. Please review the WHO Foundation funding criteria prior to submitting a request. The WHO Foundation adheres to the grant guidelines set forth when reviewing applications. Please do not apply if you do not meet the qualifications. Please include a self-addressed stamped envelope for acknowledgement of receipt of grant.
